Incident Reporting Forms

Lewis and Clark Community College cares about the safety, health, and well-being of its students, team members and community and maintains the safety of the campus community through positive, proactive, and practical risk assessment and intervention. Please read the information below to learn more about when and how to report a concern.

Students

This form should be used by students to report academic integrity issues, potential code of conduct violations, concern for another student's mental or physical health, financial aid issue, enrollment issues, sexual misconduct or an issue involving a college team member, another student or a visitor on campus..

Team Members

This form should be used by team members to report academic integrity problems, code of conduct violations, concern for a student's mental or physical health, issues of concern or possible Title IX violations.

Community Members

This form should be used by community members to report issues with a campus team member or property, witnessing a crime on campus or wishing to alert Lewis and Clark officials about a community concern.
